1. Fill out your information as usual

2. In the “Firm/Business or Agency” box, type “Self”

3. In the Title box type “Self”

4. In “Persons, groups, firms represented in this appearance” box type “Self”

5. 𝗛𝗲𝗿𝗲 𝗶𝘀 𝘁𝗵𝗲 𝗲𝘅𝘁𝗿𝗮 𝘀𝘁𝗲𝗽 -> Under Section III. Position it says, “Description” there is a drop down menu to the right of it. The default says, “Original Bill”. Click it, and select “House Amendment 1”. (It might also say House Committee Amendment 1)

6. Select 𝗢𝗽𝗽𝗼𝗻𝗲𝗻𝘁

7. Select “Record of Appearance Only”

8. Agree to the terms

9. Click "Create Slip" to submit witness slip.
